Examination of the pupil provides an opportunity to detect disturbances in the autonomic innervation of the eye. The pupil is frequently affected in patients with generalized autonomic neuropathies. This literature review confirms a high prevalence of sympathetic deficits and parasympathetic deficits in acute or subacute dysautonomia, diabetes, amyloidosis, pure autonomic failure, paraneoplastic syndromes, Sjgren syndrome, familial dysautonomia, and dopamine beta-hydroxylase deficiency. It confirms the relative scarcity of a pupil abnormality in patients with multiple system atrophy. There are difficulties in clinical diagnosis of pupil abnormalities and interpretation of pupil pharmacologic tests, particularly when combined sympathetic and parasympathetic deficits are present.

Rembrandt van Rijn is not the only artist to catch the critical eye of Queen's benefactor Alfred Bader. On Sept. 27 at Ellis Hall auditorium, Dr. Bader described a lifetime passion in a lecture entitled The Joy of Collecting for Queen's. Earlier that day, the university announced a donation by Dr. Bader and his wife Isabel Bader of a second Rembrandt painting to the Agnes Etherington Art Centre. The following are excerpts from the lecture. For many, many years, I have been truly intrigued by the works of Jan Lievens. Jan Lievens was a contemporary not a student a contemporary of Rembrandt. They probably shared a studio in Leiden in the 1620s. And then Lievens went to England and from England to Antwerp and back to Amsterdam. Rembrandt went straight to Amsterdam. I believe that Queen's will own no less than 11 paintings by Jan Lievens. Many of these I was able to purchase very inexpensively. One of my two favourites is this called Rembrandt's Mother: the most expensive. I remember walking with Isabel to the Sotheby auction in New York. And I said to Isabel, "Isabel, would you divorce me if I spent over a million dollars for that painting?" And luckily, she said no. And luckily, I didn't have to pay anywhere near a million dollars. Though in the last few years, good paintings by Lievens have brought many millions. Today, a painting like that, now at Queen's you can see it at the Agnes would bring about some- Profile head of an old thing like million. The man was truly a great woman Rembrandt's painter. And you can see why somebody wanted to over-paint that skull: not just because it's a skull, but because this fellow is actually poking his finger into the nasal cavity. Not everybody wants that in their dining room. I hope you like the picture, which is at the Agnes. Now, I sent a photograph to the greatest expert in Dutch paintings, Mr. Wolfgang Stechow at Oberlin College, Ohio ; , a very good friend. And I didn't hear from him. And so after two weeks, I called him, and I said, "What do you think of that portrait?" And he said, "That's a stunning portrait. Self-portrait with Skull, Who do you think painted it?" And I said, "Wolf, c. 1661 by Michael I was hoping that you would tell me that this is Sweerts detail ; . by Michael Sweerts." Long silence. You know what long silences from professors mean. And then he said, "Alfred. Understand what you're saying. It's intense. It's about 1660. But you can't go around the world calling every intense portrait a Michael Sweerts. He was a very great painter." And I said, "Wolf, look. You have a famous Sweerts self-portrait at Oberlin. Allow me to send you my painting and then hang the two paintings side by side. And tell me." And my friends, I must tell you, except for a few letters that Isabel sent to me in 1949, no letter has given me as much pleasure as a special-delivery letter from Wolf Stechow saying, "Alfred, we now have the two paintings side by side. There's no question. Both are by Michael Sweerts. Yours is in better condition. Both are self-portraits.
In the previous evaluation of hydroxyzine, it was noted that the ideal anti-arrhythmic agent should be effective in arrhythmias of various types and etiologies, and possess a wide margin of safety. When acute abnormal rhythms require rapid treatment, such a drug should restore normal sinus rhythm quickly. The idea anti-arrhythmic agent should also be suitable for long-term prophylactic therapy. Hydroxyzine oral, t I. M., I.V.tt ; was reported to be clinically effective in treatment of acute arrhythmias, particularly those which disturbed ventricular rhythm. Beneficial effects occurred most frequently in patients with ventricular premature beats, paroxysmal tachycardias auricular and ventricular ; , and in patients with ventricular extrasystoles complicating auricular fibrillation.1 The drug was found to be safe, easily administered and nontoxic.211 While the mechanism of action of hydroxyzine on cardiac rhythm is unknown to date, it appears that some direct myocardial effect may occur. It has been reported that in several patients treated with hydroxyzine, normal sinus rhythm resulted either during intravenous injection or within seconds thereafter.1 In addition, other properties of hydroxyzine contribute to its usefulness as an anti-arrhythmic agent. Since hydroxyzine is an effective, safe ataractic agent, its calming effect is of particular value in allaying the anxiety and apprehension of patients with cardiac arrhythmias, without impairing mental alertness. Hydroxyzine is not a cortical depressant and willn ot increase gastric secretion. The drug does not significantly lower blood pressure or produce quinidine-like side effects. RESULTS Diet intake, live weight and pancreas weight. The dry matter intake expressed on a live weight basis was high at the end of the suckling period and 22% higher after weaning Table 2 ; . Graham McKerrow, HIV i-Base Postexposure prophylaxis PEP ; consisting of antiretroviral medication and behavioural counselling following a potential sexual exposure to HIV does not lead to an increase in high-risk behaviour in most people, according to researchers in San Francisco. Jeffrey N Martin and colleagues conclude that their findings of "this lack of behavioural disinhibition" coupled with prior safety and feasibility data suggest that the use of PEP should be routinely considered following high-risk sexual exposures. The researchers conducted a non-randomised trial of 397 adults with high-risk sexual or drug-use exposure within the previous 72 hours. The intervention consisted of antiretroviral medication for four weeks and five counselling sessions. Participants were followed for 12 months to record repeat requests for PEP, modifications to high-risk behaviour and the acquisition of STDs and HIV. The majority of participants 83% ; did not request a repeat course of PEP. Seventy-three percent reported a decrease compared to baseline in the number of times they performed high-risk sexual activities, 13% reported no change and 14% reported an increase. Most, 85% ; had no change in the incidence of STDs, 8.5% had a decrease and 6.8% an increase. Three gay men seroconverted for HIV, none of which was associated with the presenting exposure; this represented a rate of 1.2 per 100 person years and was similar to rates in San Francisco for all gay men. In their discussion the authors write: "Direct proof of the efficacy of non-occupational PEP in preventing HIV transmission is still needed. Although we saw no instances of chemoprophylactic failure, we would not have necessarily expected to observe any HIV seroconversions associated with the presenting exposures even without the provision of PEP, given our sample size, the low per-exposure infectivity of HIV and the likelihood that some participants had contact with uninfected sources. Therefore, our data should not be taken as evidence for the efficacy of PEP in preventing seroconversion. Unfortunately, definitive ascertainment of the efficacy of PEP following a sexual exposure through randomised placebo-controlled trials will be difficult because of the large sample size required. Until direct evidence regarding efficacy is available, decisions must nonetheless be made on how to manage individuals with high-risk sexual exposures. Given the indirect evidence of efficacy gleaned from the occupational setting and from animal studies, coupled with our findings on feasibility and safety, we believe that PEP, comprising both antiretroviral medication and risk-reduction counselling, should be routinely considered following high-risk sexual exposures and amevive.

High speed GC MS a ; Injection system Sample introduction is the most critical step in high speed GC because it affects band broadening. The advantage of a PTV is that it combines a cool injection step with controlled vaporisation. The smaller i.d. liner of the PTV can also be heated or cooled more rapidly and this helps to reduce peak broadening. There was poor sensitivity of the high boiling point analytes at an isothermal injection temperature of 100C while thermally labile components decomposed at higher temperatures. On using a PTV temperature program from 100 to 250C, the decomposition of the thermally labile analytes was greatly reduced and better detection limits of the higher boilers were obtained. The ramp rate for the PTV was investigated at the various temperature programming rates from 50C min to 250C min. At low ramp rates, resolution was poor for two critical pairs. There was a marked improvement in peak separation, peak shape, area, and height at higher ramp rates. Hence, PTV was employed in all the analysis with the three columns. b ; Long middle bore column 30 m x 0.25 mm x 0.25 m ; It was observed that only the initial oven temperature has some impact on reducing analysis time but overall results of the other speeding approaches are relatively insignificant with some loss of resolution. Although increasing the ramp rate produced significant changes in analysis time, peak separation was poor and hence, faster temperature programming rates could not be implemented in this analysis. c ; Short middle bore column 10 m x 0.25 mm x 0.25 m ; An effective way to reduce analysis time is to reduce the column length. Increasing the ramping rate from the conventional 15C min to 70C min produced the most significant change in analysis time with good resolution. Combining all the speeding approaches resulted in a significant reduction in analysis time by 80%. d ; Short narrow bore column 10 m x 0.1 mm x 0.1m ; Narrow bore columns with thin films are more efficient and have better resolution. It is also possible to work at higher linear gas velocities with less loss in efficiency. Increasing the ramping rate from the conventional 15C min to 70C min produces very significant changes in analysis time. Combining all the speeding approaches resulted in a significant reduction in analysis time by 80%. Comparison of the three columns a ; Analysis time There was a significant 4.5 fold reduction in analysis time for both the short middle and short narrow bore columns but a marginal 1.5 fold reduction was obtained for the long middle bore column. b ; Column efficiency Plate height was used as a measure of the efficiency of the columns. Plate heights obtained for the 3 columns were similar. c ; Peak width and resolution The peak widths were similar for the two short columns, which were only the width of the peaks obtained for the long middle bore column. The resolution-calculations were carried out using the two critical pairs. The two short columns gave good resolution for the two peak pairs and a baseline separation of these pairs was obtained. The data indicate that the gain in speed was achieved without sacrificing resolution. d ; Repeatability There is usually a concern that high speed GC will produce peaks that are not ideal in shape with the presence of fronting and tailing, poor baseline resolution or poor precision of the retention.
