PULMONARY GRANULOMATOUS DENSITY IN SARCOIDOSISRACIAL DIFFERENCES Robert R. Burke MD, MSc * Ben Rybicki PhD Chad Stone MD Henry Ford Health System, Detroit, MI PURPOSE: African-American race has been associated with a higher disease incidence of Sarcoidosis, more severe disease at diagnosis, more extrathoracic manifestations, and chronic course. Our hypothesis is that the lung granuloma counts are higher in African-Americans compared to Caucasians. METHODS: Retrospective, cross-sectional study at a local referral center for sarcoidosis. Histopathologic evaluation of bronchoscopic lung biopsy specimens at diagnosis. The specimen slice represented that with the most intense granulomatous inflammation on 4X inspection and counts of granuloma made with 10X view. Granuloma were counted as bronchial or alveolar. Poisson log-linear modeling was utilized to control for potential confounding by stage of chest-ray, biopsy technique, gender, and number of pieces. RESULTS: The subject population n 189 ; was predominantly African-American 75% ; and female 67% ; . African-American race 65% increase; 95% CI, 49% to 89% ; , female gender 32% increase, 95% CI, 22% to 43% ; , presence of necrosis 57% decrease, 95% CI, 53% to 61% ; , chest x-ray stage II relative to I 62% increase, 95% CI, 47% to 78% ; , and stage III relative to I 136% increase, 95% CI, 114% to 161% ; were associated with lung granuloma counts. In a multivariate Poisson loglinear model, African-American race was significantly associated with increased granuloma counts, after adjustment for chest x-ray stage, age, gender, necrosis, biopsy type, and biopsy number 27% increase, 95% CI, 15% to 40% ; . When restricted to bronchial granuloma, the influence of race was more pronounced 55% increase, 95% CI, 38% to 75% ; , adjusted for chest x-ray stage, gender, biopsy type, biopsy number, and necrosis. CONCLUSION: The intensity of pulmonary granuloma intensity is significantly higher among African-Americans compared to Caucasians at the time of diagnosis. We provide a partial histopathologic explanation for the differences in disease severity at diagnosis and bronchoscopic lung biopsy differences by race. CLINICAL IMPLICATIONS: We provide a partial explanation for the differences by race seen in bronchoscopic biopsy yield studies. Future lung bronchoscopic lung biopsy studies need to control for the influence of race in design or analysis. Dexmethylphenidate hydrochloride d-MPH, Focalin ; has been shown to be effective for chemotherapy associated fatigue. We studied the effectiveness of d-MPH in treating sarcoidosis associated fatigue. METHODS: We performed a double blind randomized placebo controlled crossover trial of d-MPH in patients with sarcoidosis. Patients were seen weekly throughout the trial. After one week wash in period, patients were started on one tablet twice a day of either 5 mg d-MPH or placebo. At each weekly visit, patients completed instruments to assess fatigue: the non disease specific Functional Assessment of Chronic Illness Therapy-Fatigue FACIT-F ; and the Fatigue Assessment Score FAS ; , specific for sarcoidosis. For FACIT-F, the higher the score, the less the fatigue and the maximum score is 52. For FAS, the higher the score, the more the fatigue and the maximum score is 50. The number of pills 5 mg tablets of d-MPH or placebo ; was adjusted at each visit based on toxicity, with a maximal potential dose of d-MPH of 10 mg twice a day. After 8 weeks, the medications were stopped. After a 2 week wash-out period, patients were then crossed over to 8 weeks of the other treatment d-MPH or placebo ; . RESULTS: Ten sarcoidosis patients were enrolled in this study: 8 females and five African Americans. All patients were receiving systemic therapy for their sarcoidosis. The results of the FAS and FACIT-F scores were compared for the last 4 weeks of d-MPH versus placebo. Patients experienced less fatigue while on d-MPH, with the FACIT-F score being significantly higher d-MPH 28.5 7-52 ; Median range Placebo 25 4-46 ; , p 0.002 ; and the FAS score significantly lower d-MPH 28 8-43 Placebo 34 9-45 ; , p 0.005 ; . The major complaint was insomnia, which responded to lower afternoon drug doses. CONCLUSION: Treatment with d-MPH was associated with a significant improvement in sarcoidosis associated fatigue.
