Monoclonal antibody chemotherapy is a promising therapy in patients with acute myeloid leukemia AML ; . Gemtuzumab Ozogamicin GO ; consists of a anti-cancer antibiotic, calicheamicin, linked to a recombinant humanized murine antiCD33 monoclonal antibody. A few phase II studies have showed the efficacy and safety of GO, used as single agent or in combination with other drugs, in patients with de novo or first relapse AML. We treated successfully an elderly AML patient, previously refractory to two lines of standard chemotherapy, with a combination GO-cytarabine obtaining a complete remission with incomplete platelet recovery CRp ; . The novelty of this report consists, in our opinion, in the obtainement of a CRp in a primary refractory adult case. Outcome in refractory leukemia is extremely poor and the only possible curative option to date has been allogeneic stem cell transplantation. However, better results are possible if a complete remission is obtained before trasplantation. Our results, if confirmed on a larger series of patients, could open a new therapeutic role for GO, suggesting its use in refractory patients as a component of intensive regimens in order to induce reinforce a complete remission before allogeneic stem cell or autologous transplantation in this poor prognostic group of AML patients.

One such agent, Mylotarg gemtuzumab ozogamicin ; , a conjugate of the anti-CD33 antibody with the highly potent cytotoxic drug, calicheamicin, has gained Food and Drug Administration approval for treatment of CD33-positive acute myeloid leukemia in patients over age 60 in first relapse 2 ; . Other drug-MAb conjugates are currently in development for the treatment of various solid tumors. Pursuing a similar strategy of using a supertoxic drug, a MAb conjugate of a maytansinoid drug C242-DM1 ; was described, and shown to eradicate large colon tumor xenografts in athymic mice 3 ; . Liu et al. 3 ; contended that MAb conjugates of conventional chemotherapy drugs, such as doxorubicin dox; Ref. 4 ; or vinblastine 5 ; , had inadequate potency to elicit a clinical effect, and showed that C242-DM1 cured animals at a 50-fold lower dose than an earlier-described dox-BR96 MAb conjugate 4 ; . Acyclovir is a synthetic purine nucleoside analog derived from guanine. The drug differs structurally from guanine by the presence of an acyclic side chain. Acyclovir is commercially available for parenteral use as the sodium salt and for oral use as the base. Potency of commercially available acyclovir sodium powder or concentrate for injection is expressed in terms of acyclovir. Acyclovir occurs as a white, crystalline powder and has a maximum solubility of 2.5 mg mL in water at 25C. The drug has pKas of 2.27 and 9.25. Commercially available acyclovir sodium powder for injection occurs as a white, crystalline, lyophilized powder.
