In vitro and in vivo studies show that drug carriers are expressed in the BBB and in the CSB. They represent major determinants of toxicity and clinical outcome related to drug response. Understanding the functional significance of membrane transporters in the BBB and in the CSB provides further opportunities to improve drug delivery to the CNS. We propose that the role of transporter proteins should be studied at an early stage of CNS drug development, as there are in vitro methods such as cell cultures to achieve this purpose. Knockout animals are valuable in vivo models, but in vivo methods in humans are few. Direct in vivo determinations of drug concentration and effective transporter function into the brain remain particularly challenging, as invasive techniques are necessary. Neuroimaging techniques should be helpful, since molecules can be measured by positron emission tomography PET ; or by magnetic resonance imaging spectrometry. For example, the latter can be used to assess the pharmacokinetics of some fluoride-containing molecules in the brain. Although several members of the membrane transporters present in the BBB have been characterized in detail, numerous questions remain open. Firstly, the determination of detailed tissue expressions and in vivo studies of carriers with better specificity are required to target more efficiently therapeutic agents into the CNS and into other organs. Secondly, in order to enhance the potential clinical implications of drug transporter polymorphisms and interactions, the development of specific inductors and inhibitors may represent promising strategies. Thirdly, future delivery procedures include the use of prodrugs, drug-targeting vector conjugates, or liposomes tagged with targeting vectors to elude physiological barriers. Drug transporter protein studies provide insight into the mechanisms of resistance, treatment failure, and interindividual response to neurological and psychiatric medication. Membrane transporter proteins are not only CNS gatekeepers, but represent determinant partners in CNS drug development strategies. Exploring the functional significance of membrane transporters in drug delivery to the CNS is essential for the treatment of neurological and psychiatric disorders.
Many thanks to all the members of the League--On Board and Off Board--for their work during the Presidential election year and the 79th Texas State Legislature. This challenging year was preceded by many years of developing and refining League procedures; however, we still had many demands. Just as all four wheels of an automobile must be fully operable to perform at its optimum, the four vice presidents task areas need to be fully operable for the LWV to perform at its optimum. Much applause and appreciation goes to all the volunteer efforts that help the LWV perform optimally. Special thanks goes to the Executive Committee members for their efforts to make a successful year. Kristen Castellanos has done an outstanding job as Secretary for the last three years. This year she was also Chair of the 85th Anniversary Party and an observer at the Dallas County Commissioners' Court. Jane Davis came back on the Board, after a short break, to the office of Treasurer and has done a great job. Judi Stewart, Program Vice President, did a wonderful job planning the 20042005 unit and general meeting programs. With so many critical issues, she planned meetings with many topics that needed to be covered so that LWV and the community members could be informed participants in the political process. Janie Metzinger, Community Relations Vice President, served during the most difficult period, the five months before the 2004 Presidential Election. She was able to serve for seven months before her work prohibited her from finishing the year. The people that worked with Janie are: Sandy Thornton, Candidate Forum Chair; Jane MacAdams, Voter Service Chair, along with Maureen Jones, Liz Wally, Evelyn Mitchell, Libbie Terrell Lee, and many more League members. Michelle Manners was Publicity Chair with many LWV activities to report. Roger Kallenberg and I developed the questions for the Voters' Guide on the web and the candidate forum questions with the issue chairs' contributions. Much appreciation goes to Organization Vice President Joanne Early and all the members who provided for the League in her area. Joanne coordinated the office with Nancy Todes as a number one helper. Lea Frailey, Agnes Ellsworth, Pat Vaughan, Ru Aaronson, Kristen Castellanos, Lucille Gropp, Julie Lowenberg and Ann Adams were volunteers with Eleanor Sutherland as Volunteer Chair. Ann Adams volunteered in the office every Tuesday all year long. Sally Garcia, League staff, was key to every effort and a delight. Many members ably assisted during the months before the 2004 election. Best wishes do not run an organization so Linda Wassenich was instrumental as Development Chair. Two members, Shirley Cooper and Virginia Kerner, did arrangements for the general meetings. Dottie Box oversaw membership as chair with help from Nancy Todes. Nancy was a tremendous asset in a number of areas as she also planned the annual meeting. Alix Mason served another year as the Publications Chair. Katherine Homan was Chair of Unit Organization. A 13 year old boy presented with a six month history of diarrhoea, abdominal pain and weight loss. He had been opening his bowels 34 times each day and had dark red blood in his motions on at least three occasions. His past medical history included asthma for which he was receiving salbutamol 100200 micrograms prn and beclomethasone 200 micrograms twice daily.
