In general, four basic methods have been routinely used to study or measure antimalarial drug resistance: in vivo tests, in vitro tests, animal model studies and molecular characterisation Bloland, 2001 ; . This thesis focuses on in vivo tests. An in vivo test is the treatment of a group of malaria patients with known doses of drug and the subsequent monitoring of the parasitological and or clinical response over time. One of the key characteristics of in vivo tests is the interplay between host and parasite. Reduced therapeutic efficacy of a drug can be masked by immune system clearance of parasites in patients with a high degree of acquired immunity White, 1997 ; . A high degree of acquired immunity is seen in populations in holo-endemic areas for malaria, such as the study populations of this thesis. A standardised method for assessing efficacy of chloroquine in vivo was first developed in 1965. This was later revised twice, in 1967 and 1972 WHO, 2003a ; . This method was quite demanding and over time, the focus of research changed from the initial parasitological focus to clinical outcome and efficacy in the most vulnerable patient groups. Therefore, a workshop was arranged that resulted in the development of a new standardised protocol for assessing efficacy of antimalarial drugs WHO, 1996 ; . This protocol emphasised clinical response over parasitological response and after several years of use, a new workshop was arranged to revise it. In 2003 the revised protocol was published, defining drug efficacy as combined parasitological and clinical outcome WHO, 2003a ; . Both of the protocols, from 1996 and 2003, have been used in this thesis!

In 150 examinations, there were no serious complications. One bladder and two intestinal punctures occurred, but the patients experienced no adverse effects. Prior to the use of contrast agents for needle localization, nine patients had extraperitoneal injection of gas. Radiographs in these patients showed an `empty' or gas-filled pelvis without internal genital structures. Postprocedural pain, especially shoulder pain in older children, was diminished by removing gas at the end of the procedure. While we could not determine the incidence of this pain in this retrospective analysis, it was app reciable enough to warrant premedication of later patients in the series. Although gas embolism was thought possible, we had no cases and the risk was considered negligible because nitrous oxide is highly soluble in body fluids. 2.7.4 Observations of the Sun's magnetic field during the recent solar maximum T.R. Sanderson, T. Appourchaux and US colleagues have analysed a comprehensive set of solar data to show how the magnetic field of the Sun and the associated coronal holes varied through the last two solar cycles, and in particular the recent solar maximum Sanderson et al., 2003 ; . The behaviour of the dipole term and the quadrupole term were analysed, and a new way of visualising them was developed. This shows the manner in which the dipole rotates once every 22-year cycle, and also how the quadrupole varies around solar maximum. Combining coronal hole outline data with the magnetic field data reveals that the sites of the coronal holes move across the Sun's disc in sympathy with the motion of the poles of the dipole, and also that when the poles of the quadrupole become apparent, the coronal holes break up into smaller, like-polarity groups of holes. The way in which this influences the distribution of highspeed solar wind flow in the heliosphere, how it and rifapentine.

The case for international public goods, separable into global and regional public goods, arises from the collective action problems and strong externalities that are associated with such transnational challenges and threats to stability as financial contagion, rising inequality, the spread of communicable diseases, cross-border crime and conflict, or the degradation of shared natural resources. International public goods include the knowledge, standards and rules required to address these challenges, the institutions that monitor and enforce the rules, and the benefits that arise and are shared indiscriminately among countries. Transnational development challenges -and the hard edges of globalization- are becoming more visible as countries become more interdependent and are more actively pursuing integration. Controlling these challenges and harnessing the opportunities of globalization and integration requires international cooperation. Sovereign nations must work together, and in the process must assert their sovereignty in new ways, that is, through contributions to mutually beneficial interdependence. However, collective action among sovereigns is difficult to achieve. Barrett forthcoming ; explains why this is so, and why supplying global and regional public goods is more difficult than creating national and local ones. The production of national and subnational public goods is facilitated by the institutions and the taxation authority of the state. There is no comparable arrangement to foment the supply of international public goods. Different countries and their citizens value the benefits of public goods differently. This makes coordinating their production demanding World Bank [2001a] ; .1 Experience shows that international organizations and official development assistance can help. International organizations can act as catalysts for collective action.
